Summer Camp

Summer camp is right around the corner and NOW is the time to get physicals and health forms filled out. We need the health forms by June 3 to verify all of the information and get them to the camp staff before we arrive. No one will be allowed to go to camp without all health forms completed.

**** HEALTH FORMS DUE JUNE 3*****

Health forms for all participants of Summer Camp are due to Mr. Kerr by Monday, June 3, 2024. This applies to both youth and adults. This *does* require a physical and the original form is required. The camp will not return the forms to us but we will make copies. The form is here: https://filestore.scouting.org/filestore/HealthSafety/pdf/680-001_ABC.pdf

Note: There is a checkbox on Part B2 that says “Non-Prescription medication administration is authorized”. This requires a parent signature. This is extremely important to sign so that the medical personnel can give your child over the counter medicine such as Advil. If there are any restrictions to these medications, please list them in the exceptions.

  • For instance, if the onsite medical personnel is authorized to give your child over the counter medicine, check “Yes” and sign with no exceptions.

**** DIETARY NEEDS FORM DUE JUNE 3****
